Estudio de la maduración de los ritmos circadianos de temperatura y movimiento en prematuros como marcadores precoces del desarrollo neurológico

Abstract

Objetivos: El objetivo principal es analizar los parámetros identificadores de los ritmos de temperatura y actividad en niños prematuros de 30-34 semanas y niños nacidos a término y compararlos con los resultados obtenidos mediante test psico-motores y de comportamiento a 6, 12 y 18 meses de vida, para su validación como marcadores del desarrollo neurológico en niños. Metodología: Se reclutaron 24 niños prematuros con una edad gestacional comprendida entre las 30 y 34 semanas y 37 niños recién nacidos a término sanos. Se realizó un reconocimiento los primeros días de vida que incluyó exploración física general y somatométrica y un seguimiento hasta los 18 meses de vida con revisiones al mes, a los tres, seis, doce y dieciocho meses de edad (corregida para los prematuros y cronológica para los controles). En las revisiones del mes, los tres y seis meses se entregó a los padres unos sensores acoplados en unos calcetines para el registro de los ritmos de actividad y temperatura durante su actividad habitual en su casa. En las revisiones de los seis, doce y dieciocho meses se pasó el test de Bayley para el desarrollo (escalas motora y mental) por parte de dos psicólogas. El análisis de los ritmos circadianos se realizó mediante un software específico calculando distintas variables que se utilizan en Cronobiología: tests paramétricos como el análisis de Fourier (armónicos P1 y P1/Pu) y no paramétricos como la media, la estabilidad interdiaria (IS), la variabilidad intradiaria (IV), la amplitud relativa (RA) el índice de concordancia de sueño I<O o el índice de función circadiana (CFI). Resultados y conclusiones: Los ritmos circadianos de los prematuros son menos robustos que los que muestran los controles de la misma edad cronológica, caracterizándose por presentar una menor regularidad, mayor fragmentación y menor amplitud que los controles A los tres meses de edad cronológica, los prematuros presentan una media de temperatura periférica mayor y claras diferencias en todas las variables que indican robustez del ritmo (IS, IV, RA, P1/PU), tanto de temperatura, como de actividad y sueño. Sin embargo, la mayoría de estas diferencias desaparecen cuando se corrige la edad de los prematuros, con la excepción de la persistencia del adelanto de la hora del sueño nocturno al mes y los tres meses y de la hora de la siesta a los seis meses observada en los prematuros. Existe una maduración diferencial entre los ritmos de temperatura y actividad, alcanzando el de temperatura un patrón de predominio circadiano a los 3 meses respecto al de actividad, donde esta maduración se alcanza a los seis meses. Así mismo, podemos observar cómo en todas las variables registradas, la fragmentación del ritmo disminuye de forma visible entre el mes y los tres meses con la disminución de la variabilidad intradiaria mientras que la estabilidad del ritmo continúa aumentando progresivamente hasta los seis meses. El desarrollo psicomotor medido mediante el test de Bayley muestra mayores diferencias a los 18 meses que a edades más tempranas, obteniendo los prematuros una puntuación más baja que los controles a nivel cognitivo y motor. La pendiente de maduración del índice de función circadiana (CFI) se correlaciona de forma significativa con la puntuación motora a los seis meses de todos los niños y de forma casi significativa a los doce meses, por lo que a mayor maduración del ritmo circadiano del sueño, mejor desarrollo motor en el primer año de vida. Sería interesante corroborar la asociación de estos marcadores del ritmo circadiano con medidas del neurodesarrollo en edades posteriores del niño para confirmar las persistencia de estas señales cuando el neurodesarrollo ya esté más consolidado


The main objective of this study is to analyze the parameters of skin temperature and activity rhythms in premature children of 30-34 weeks and in children that are not preterm and compare both of them with the results obtained by psycho-motor and behavioural tests at 6, 12 and 18 months of age, in order to use them as neurodevelopmental markers in children. Methodology: On one hand, 24 premature children with a gestational age between 30 and 34 weeks were recruited and 37 not preterm, new-born children, on the other. They were examined during their first days of life focusing on general and somatometric physical examination as well as longitudinal recordings were performed up to 18 months of age, at 1, 3, 6, 12 and 18 months of age (corrected age for premature children). At 1, 3 and 6 months of age, parents were given skin temperature and motor activity sensors located in baby socks specifically designed for the study and their use at home. During the examination of the six, twelve and eighteen months of age, they passed the Bayley test for development (motor and mental scales) by two psychologists. The analysis of the circadian rhythms was performed using a specific software, calculating different variables that are used in Chronobiology: parametric tests such as Fourier analysis (P1 and P1 / Pu harmonics) and non-parametric tests such as the average, interdaily stability, intradaily variability (IV), relative amplitude (RA), the sleep concordance index I <0, or the circadian function index (CFI). Results and conclusions: The circadian rhythms of premature children are less robust than those shown by tests of the same chronological age, characterized by less stability, greater fragmentation and less amplitude. At 3 months of chronological age, premature children have a higher average peripheral temperature and clear differences in all variables that indicate the robustness of the rhythm (IS, IV, RA, P1 / PU), both temperature, activity and sleep. However, most of these differences disappear when the age of premature babies is corrected by post-menstrual age, with the exception of the steady progress of the night sleep at 1 and 3 months and the nap time observed at 6 months in premature ones. There is a different maturation process between the rhythms of temperature and activity, reaching the temperature one a pattern of circadian rhythm at 3 months. In this way, the activity reaches the circadian rhythm at six months. Likewise, we can observe that in all the variables recorded, the fragmentation of the rhythm decreases between the first month and the three months of age, within the decrease of intraday variability, while the stability of the rhythm continues to increase progressively up to the six months of age. The psychomotor development measured by the Bayley test shows greater differences at 18 months than at earlier ages, with prematures achieving a lower score than the test results at the cognitive and motor levels. The maturity gradient of the circadian function index (CFI) correlates significantly with the motor score at 6 months of all children and slightly significantly at 12 months, therefore the higher maturation of circadian sleep rhythm, the better motor development in the first year of life. It would be interesting to correlate the association of these circadian rhythm markers with neurodevelopmental measures in later ages of the child to confirm these signals continue when the neurodevelopment is already more consolidated.
