Abstract

Les bronquièctasis són dilatacions anormals i irreversibles dels bronquis amb inflamació de la paret bronquial i alteració de l’epiteli ciliar que dificulten l’eliminació de secrecions i faciliten la infecció bronquial. La fisioteràpia respiratòria és essencial per l’aclariment mucociliar encara que l’evidència dels seus beneficis és escassa. Objectius: Crear i validar una versió espanyola del Leicester Cough Questionnaire (LCQ) per poder-la utilitzar com a variable d’estudi. Avaluar el grau de coneixement i de compliment de les tècniques d’aclariment mucociliar en pacients amb bronquièctasis en el nostre mitjà. Avaluar l’eficàcia a curt i llarg termini de la tècnica de drenatge espiració lenta perllongada amb glotis oberta en decúbit lateral (ELTGOL) en bronquièctasis en un estudi randomitzat i controlat amb placebo. Material i mètodes: Creació de la versió espanyola del LCQ amb un protocol estandarditzat i validar-la en pacients amb bronquièctasis en un estudi multicèntric. Per la fiabilitat es va analitzar la consistència interna i el canvi als 15 dies en fase estable. Es va analitzar la resposta al canvi entre la fase estable i la primera agudització. Per la validesa es va avaluar la validesa convergent amb la correlació del qüestionari de St George’s ( SGRQ) i la validesa discriminant. Es va analitzar el coneixement de les tècniques de fisioteràpia respiratòria en pacients amb bronquièctasis en el nostre mitjà. Pacients adults amb bronquièctasis i expectoració crònica que no estaven realitzant cap tècnica de fisioteràpia van ser randomitzats a realitzar ELTGOL o placebo dos cops al dia durant un any. Variable principal: volum d’esput expectorat durant la intervenció i a les 24h. Variables secundàries: aguditzacions, la puntuació dels qüestionaris de la versió espanyola del LCQ i del SGRQ, funció pulmonar, capacitat d’exercici, inflamació sistèmica, compliment i efectes secundaris. Resultats: Es va crear la versió espanyola del LCQ amb uns resultats excel·lents. Per la validació es van incloure 259 pacients amb bronquièctasis de diferent gravetat. La consistència interna va ser alta per la puntuació total i bona pels diferents dominis (α de Cronbach 0,86-0,91). El test-retest va mostrar un coeficient de correlació intraclasse de 0,87 per la puntuació total. La puntuació mitja a la visita 1 va disminuir a l’inici de l’agudització (p<0,001). La correlació entre el LCQ i el SGRQ va ser -0,66 (p<0,01). Les diferències en la puntuació total del LCQ entre els diferents grups de gravetat van ser significatives (p<0,001). El coneixement i adherència de les tècniques de fisioteràpia respiratòria es va analitzar en 46 pacients, els 24 (52.2%) que havien estat prèviament valorats per un fisioterapeuta tan sols a 4 se’ls havia ensenyat una tècnica efectiva. 22 pacients van ser randomitzats al grup ELTGOL i 22 al grup placebo. Els pacients del grup ELTGOL van expectorar més volum d’esput durant la intervenció i a les 24hores (p<0,001) tant a l’inici com al final de l’estudi. El número d’aguditzacions va ser menor en el grup ELTGOL (p=0,042). En el grup ELTGOL la puntuació del SGRQ va millorar als 6 mesos ( p<0,001) i l’impacte de la tos va disminuir (p=0,004). Conclusions: La versió espanyola del LCQ és capaç de discriminar la gravetat de la malaltia, és sensible al canvi en el cas d’aguditzacions i és fiable per utilitzar en pacients amb bronquièctasis. El coneixement de les tècniques d’aclariment mucociliar en el nostre mitjà és baix. La tècnica ELTGOL realitzada dos cops al dia durant un any en pacients amb bronquièctasis augmenta el volum d’esput expectorat durant i després del procediment, i això s’ha associat amb menys aguditzacions, amb una milloria de la qualitat de vida i en una reducció de l’impacte de la tos.


Bronchiectasis is an abnormal and irreversible dilatation of the bronchi with inflammation of the bronchial wall and ciliary epithelium dysfunction that leads in difficulty of the airway drainage and promotes bronchial infection. Respiratory physiotherapy is essential in airway clearance despite there being little scientific evidence regarding its benefits. Objectives: Create and validate a Spanish version of the Leicester Cough Questionnaire (LCQ) to use it as an outcome. Evaluate level of knowledge and adherence of airway clearance techniques in the area of our study. Evaluate short and long-term efficacy of the slow expiration with the glottis opened in the lateral posture (ELTGOL) technique in patients with non-cystic fibrosis bronchiectasis in a randomized-placebo controlled trial. Materials and Methods: Create a Spanish version of the LCQ with a standardized protocol and it’s validation in bronchiectasis patients in a multicentric study. Reliability was analysed calculating internal consistency and test-retest changes at 15 days of stable state. Responsive to change was calculated comparing stable state and the first exacerbation. Validity was calculated using convergent validity analysing correlation with St. Georges Questionnaire (SGRQ) and the discriminant validity. The level of knowledge of airway clearance techniques in bronchiectasis patients was evaluated. Adult patients with bronchiectasis with chronic sputum who were not practising any regular physiotherapy were randomized to perform ELTGOL or placebo twice daily during 12 months. Primary outcome: sputum volume during intervention and 24-hour post-intervention. Secondary outcomes: exacerbations, the LCQ and SGRQ scores, pulmonary function, exercise capacity, systemic inflammation, adherence, and side effects. Results: the Spanish version of the LCQ was created with excellent results. For the validation 259 patients with different severity were included. Results from reliability showed a high internal consistency (Cronbach’s a values ranging 0.86–0.91). The test–retest reliability showed an Intraclass Correlation Coefficient value of 0.87 for the total score. The mean LCQ-Sp score at visit 1 decreased at the beginning of an exacerbation (p< 0.001). The correlation between LCQ and SGRQ scores was -0.66 (p< 0.01). The differences in the LCQ total score between the different groups of severity were significant (p< 0.001). Level of knowledge and adherence of airway clearance techniques was analysed in 46 patients, of these, 24 (52.2%) had previously been seen by a physiotherapist although only 4 had been taught an effective airway clearance technique. 22 patients were randomized in the ELTGOL group and 22 in the placebo group. Patients in the ELTGOL group obtained more sputum volume during the session and 24-h after (p<0.001) even at baseline at the end of study. There were fewer exacerbations in the ELTGOL group (p=0.042). The ELTGOL group showed an improvement at 6 months in the total score of the SGRQ (p<0.001) and the impact of coughing decreased (p=0.004). Conclusions: The Spanish version of the LCQ is able to discriminate disease severity, is responsive to change in the event of exacerbations and is reliable to use in those patients. In the area of our study, knowledge of airway clearance techniques and adherence to them is very poor. Twice-daily ELTGOL technique over one year managed to increase sputum volume expectorated in patients with bronchiectasis, and this was associated with fewer exacerbations, an improvement in quality of life and a reduction in the impact of coughing.
