L'exposició a pacients amb tuberculosi (TB) activa està considerada com a un factor de risc laboral, i la malaltia tuberculosa es considera una malaltia professional en els treballadors sanitaris. Malgrat això, existeixen poques dades en relació a la seva magnitud en el nostre país. L'objectiu d'aquesta tesi ha estat con ixer la magnitud de la infecció i de la malaltia tuberculosa en el personal sanitari de l'Hospital Germans Trias i Pujol en les darreres 2 dècades, i analitzar els factors de risc associats. D'altra banda s'han avaluat les proves de determinació in vitro d'alliberació d'interferó gamma (IFN-γ) pel diagnòstic de la infecció tuberculosa (IT) en el personal sanitari.
S'han analitzat els registres dels treballadors amb prova de tuberculina (PT) realitzada a la Unitat de Medicina Preventiva des de l'any 1988 fins l'any 2007 (2149 treballadors). La prevalença de PT positiva basal ha estat del 25.7% (interval de confiança del 95% (IC 95%) de 23,8 a 27,4%) i ha estat superior en el període 1988-1992 (44,2%) e inferior en el període 2003-2007 (15,8%). Dels treballadors amb PT negativa basal s'ha realitzat un seguiment posterior a 614 treballadors sanitaris. La taxa d'incidència d'IT ha estat de 3,8 per 100 persones-any (IC 95% de 3,7 a 4,0). Els treballadors de les àrees d'alt risc tenen un risc d'IT 1,55 vegades superior que els de les àrees de baix risc (IC 95% de 1,05-2,27). També s'ha estimat la incidència de malaltia tuberculosa en el personal sanitari de l'hospital en el mateix període, detectant-ne 22 casos de tuberculosis. L'estament professional més afectat ha estat el dels metges residents (36,4%), i pràcticament el 50% casos treballaven a urgències. La incidència anual per períodes quinquennals ha variat de 164,18 per 100.000 habitants en el període 1988-1992 a 12,01 en el període 2003-2007. S'ha observat un excés d'incidència de tuberculosis, en comparació a la població general de Catalunya, en el període 1988-1992 (Raó d'incidència estandarditzada de 2,64; IC 95% de 1,44 a 4,78).
S'ha analitzat l'acceptació i el compliment amb el tractament de la infecció tuberculosa (TIT) en 112 treballadors amb PT positiva. Van acceptar iniciar 36 treballadors, dels que 30 van realitzar com a mínim 6 mesos de TIT (compliment del 26,8%), amb un major compliment en els treballadors amb conversió recent, els homes, els metges i els treballadors del torn de nit.
Per últim, s'han avaluat les proves de detecció in vitro d'alliberació d'IFN-γ (Quantiferon Gold In Tube i T-SPOT.TB) pel diagnòstic de la IT en 147 treballadors sanitaris. En els 52 treballadors amb una PT prèvia negativa o realitzada per primera vegada, la concordança valorada amb l'índex Kappa (k) entre T-SPOT.TB i QFN-G-IT amb la PT, va ser de 0,754 i 0,929 respectivament. Quan es van incloure les persones amb PT positiva prèvia, la concordança (K) va disminuir a 0,321 i 0,288 respectivament. Es va trobar una associació entre el resultat positiu en una prova d'IFN-γ i el grau d'exposició.
Es pot concloure que el risc d'infecció i de malaltia tuberculosa en el personal sanitari ha disminuït al llarg del període estudiat. Una de les causes es pot atribuir a la disminució de la incidència de TB en la població atesa l'hospital i a la millora en les mesures de prevenció i control de transmissió nosocomial realitzades. Les proves de detecció in vitro de la IT poden ser de major utilitat en els treballadors sanitaris, a qui està indicat realitzar proves seriades, i al estar més associades amb l'exposició laboral, poden ser més indicatives d'infecció recent, important a l'hora de valorar el TIT.

Exposure to patients with tuberculosis (TB) is considered an occupational risk, and the disease is considered an occupational disease in health workers. However, little data exist about its magnitude in our country. The objective of this thesis was to determine the extent of infection and TB disease in health personnel of the Hospital Germans Trias i Pujol in the last 2 decades, and analyze the associated risk factors. On the other hand, tests have been evaluated in vitro determination of release of interferon gamma (IFN-γ) for the diagnosis of tuberculosis infection (TI) in health care workers.
We analyzed the records of workers with tuberculin skin test (TST) made to the Preventive Medicine Unit from 1988 until 2007 (2149 workers). The prevalence of positive baseline TST was 25.7% (95% confidence interval (95% IC) from 23.8 to 27.4%) and was higher in the period 1988-1992 (44.2%) and lower in the period 2003-2007 (15.8%). Workers with negative baseline TST tracked back to 614 health workers. The incidence rate of TI was 3.8 for 100 person-years (95% CI 3.7 to 4.0). Employees of high-risk areas of TI risk are 1.55 times higher than areas of low risk (95% CI 1,05-2,27). We have also estimated the incidence of tuberculosis in health personnel of the hospital during the same period, and we detect 22 cases of tuberculosis. The most affected professional category were medical residents (36,4%) with the emergency service being the work place with the highest risk. The annual incidence varied from 164.18 per 100,000 inhabitants in the period 1988-1992 to 12.01 in 2003-2007. We observed an excess incidence of tuberculosis, compared to the general population of Catalonia in the period 1988-1992 (standardized incidence ratio 2.64, 95% CI 1.44 to 4.78).
We have analyzed the acceptance and compliance with the treatment of tuberculosis infection (TTI) in 112 workers with positive TST. 36 workers was initiated, of which 30 were at least 6 months of TTI (compliance 26.8%), with a greater compliance in workers with recent conversion, in men, in medical doctors and in night shift workers.
Finally, we evaluated the tests in vitro release of IFN-γ (Quantiferon Gold In-Tube and T SPOT.TB) for the diagnosis of TI in 147 health care workers. In 52 workers without a previous positive TST, the concordance using the Kappa coefficient (k) between T-SPOT.TB ,QFN-G-IT and the TST, was 0.754 and 0.929 respectively. When we included those with a previous positive TST, the concordance (K) decreased to 0.321 and 0.288 respectively. We found a positive association between a positive and occupational job degree exposure.
We can conclude that the risk of infection and tuberculosis in health care workers has declined over the period studied. One of the reasons can be attributed to the reduction in the incidence of TB in the population attending the hospital and the improvement in prevention and control of nosocomial transmission. The IFN-γ test in vitro can have greater utility in health care workers, because periodical TST has been recommended, and the results indicate that the IFN-γ test are a useful tool for detecting recent infection, important when evaluating the medical therapy in this population.
