Abstract

El accidente cerebrovascular (AVC) es un problema de salud en nuestra sociedad, representa la tercera causa de muerte en el Mundo Occidental, y la primera de mortalidad global en España. Además de ser una patología de gran incidencia, tiene una gran prevalencia. Si bien el AVC es una causa importante de muerte, es más invalidante que mortal. Consecuencia de la asociación entre AVC y el aumento del nivel de discapacidad permanente, los costes asociados al AVC son extremadamente elevados. La hospitalización aguda y el ingreso en una institución son una parte importante de los gastos económicos por AVC. Es por esta razón que consideramos interesante estudiar en nuestro medio, los factores que pueden estar relacionados con el destino al alta hospitalaria del AVC, y tratar de buscar una asociación entre estos y la necesidad de ingreso en una institución.<br/><br/>Contribuciones que aporta la tesis: <br/>El ingreso en institución del AVC agudo puede estar mediatizado por múltiples factores, y hay numerosos estudios publicados que intentan explicar cuáles de ellos tienen más relación con el grado de ingreso en institución al alta. En nuestro medio no hay datos , por lo que la hipótesis de nuestro trabajo es identificar los factores que influyen en el ingreso en institución del paciente con AVC.<br/><br/>Objetivos: <br/>1) Estudiar las variables clínicas y demográficas, al ingreso del AVC, que pueden predecir el ingreso en institución . <br/>2) Construir algoritmos de decisión para predecir el destino al alta a partir de factores de predicción hallados en el punto 1. <br/>3) Evaluar las diferencias clínicas y sociodemográficas entre los pacientes que al alta hospitalaria ingresan o no en institución.<br/><br/>Metodología:<br/>Se trata de un estudio de cohortes históricos que comprende los ingresos por AVC en el Servicio de Rehabilitación del Hospital Joan XXIII de Tarragona, en el periodo entre 1994 y 2001 incluidos. La población de estudio es de 214 pacientes consecutivos. El estudio estadístico se realiza mediante análisis descriptivo, bivariable y multivariable. En el análisis multivariable se efectúa un análisis de regresión lineal múltiple, con nivel de significación estadística de p< 0,05.<br/><br/>Conclusiones: <br/>1) Los factores clínicos y demográficos de mayor capacidad de predicción del ingreso de un AVC al alta hospitalaria son: edad, sexo, capacidad funcional, soledad, barreras arquitectónicas, comorbilidad y hemiparesia izquierda. <br/>2) El algoritmo de decisión de destino al alta se construye con las tres variables de mayor capacidad de predicción: capacidad funcional al ingreso, edad y soledad. Este algoritmo contempla ocho grupos de probabilidad diferentes que varían del 3,6 % al 79,6 %. <br/>3) De las diferencias clínicas entre los pacientes que al alta del ingreso en Rehabilitación ingresan o no en institución, resaltamos que los pacientes con mala capacidad funcional al ingreso y edad inferior a 75 años, aquellos que ingresan en institución tienen una capacidad funcional final mucho peor, ganan menos funcionalidad con el tratamiento, aunque están ingresados un tiempo similar. De los mayores de 75 años y mala capacidad funcional inicial, los que ingresan en una institución tienen también una capacidad funcional peor, ganan también menos, pero no todo ello en menor medida que el grupo anterior y por el contrario están menos tiempo ingresados en el Hospital de agudos. <br/><br/>Resultados tangibles de la investigación:<br/>Mediante comunicaciones y ponencia en Congresos:<br/>1. The stroke is a common health problem in our society. It represents the third cause of mortality in the Western World and the first cause of mortality in Spain. In addition to having a high incidence rate, this pathology is also very prevalent. Although strokes are important cause of mortality, it causes more disabilities than deaths. The economic costs involved in strokes are very high due to the direct relationship between strokes and the high level of disabilities they provoke. Acute hospitalization and institutionalisation are very important parts of the economic costs of strokes. For this reason, we are interested in studying the factors which may provoke the association between these factors and the need for institutionalisation.<br/><br/>Thesis applications<br/>The institutionalisation of stroke victims can be influenced by multiple factors. While there are a lot of published estudies which attempt to explain which factors influence the degree of institutionalisation of these patients at discharge, there are none in our environment. The purpose of our work is to identify the factors which influence the institutionalisation of stroke victims.<br/><br/>Objectives<br/>1) To study clinical and demographic factors upon admission which may foretell the institutionalisation of a stroke victim.<br/>2) To build algorithms of decision to predict the future of a patient at discharge from the hospital using the prediction factors found in point 1.<br/>3) To evaluate the clinical and sociodemographic differences between those hospitalized patients who are later institutionalized at discharge and those who are not.<br/><br/>Methodology<br/>This is a historic cohort study which made use of stroke in-patients at Rehabilitation Services of Hospital Joan XXIII of Tarragona, from 1994 to 2001 inclusive. The study group is composed of 214 consecutive patients. The statistical study is carried out using descriptive, bivariable and multivariable analysis. The multivariable analysis is performed with a regressive lineal multiple analysis with a significant difference of p<0,05.<br/><br/>Conclusions<br/>1) The clinical and demographic factors which most accurately predict the institutionalisation of stroke victims are: age, sex, functional performance, living alone, architectural barriers, comorbility, and left hemiparesis.<br/>2) The algorithm of decision for discharge is constructed with the three variables which most acurately predict the future of these patients: functional performance at admission, age and living alone. This algorithm has eight groups of different probabilities of institutionalisation from 3,6% to 79,6%.<br/>3) Regarding the clinical differences between institutionalized patients and non-institutionalized patients, we stress that of those patients with low functional performance at admission and under the age of 75, those which are institutionalized tend to have a low final functional performance after treatment (they improve less with treatment), despite being hospitalized for similar periods of time. Patients over 75 years of age and with low initial functional performance also improve less with treatment, but not proportionally less than the previous group and on the contrary spend less time as inpatients in acute hospitals.<br/><br/>Results of the investigation<br/>Using the media and participating in Congresses:<br/>1.
