Resumen

Este texto es el resultado de un intento de aproximarse, desde la Antropología pero de la mano de la Epidemiología y la Salud Pública y con el foco puesto en el sistema sanitario público, a las posibles respuestas a una serie de preguntas: ¿qué factores pueden estar implicados en la aparición de desigualdades sociales en cuanto al estado de salud y en la atención sanitaria que reciben las poblaciones inmigrantes y minorías étnicas en nuestro medio? ¿hay diferencias, en este sentido, respecto al resto de la población? y ¿cómo puede nuestro sistema sanitario público dar respuesta a las necesidades de estas poblaciones? Y es el intento de responder a estas preguntas de manera acotada a un ámbito geográfico y temporal concreto: de manera específica para el ámbito español y, sobre todo, catalán, entre los años 2005 y 2010. El texto está estructurado en tres partes. La Primera Parte, titulada Aproximación a los estudios sobre salud y desigualdad, está formada por dos capítulos. Aborda las principales características de ciertas investigaciones realizadas tanto desde el ámbito de la salud como desde la Antropología sobre las desigualdades sociales en salud, diversidad e inmigración, el análisis de los fundamentos sobre los que asientan, sus posibles aportaciones y limitaciones y la exposición de los principales datos epidemiológicos disponibles en cuanto a la situación de salud y asistencia sanitaria de la población inmigrante en situación o riesgo de exclusión social La Segunda Parte, titulada El acceso a los cuidados de salud por parte de la población inmigrante: derechos y condiciones, está constituida por el Capítulo 3 y una adenda a este capítulo que responden, en su conjunto, a un intento de profundizar en la indagación sobre cuáles podían ser los factores implicados en el estado de salud y uso de los servicios sanitarios por parte de la población inmigrante en nuestro medio y en qué medida podrían relacionarse con la generación de desigualdades sociales en salud. La Tercera Parte, titulada Hacia una contrastación empírica de las ideas, recoge los resultados de una investigación etnográfica en centros y con profesionales sanitarios. La intención de esta investigación fue que permitiera, al mismo tiempo, contrastar los datos recogidos en la bibliografía en los apartados anteriores y poner a prueba las ideas iniciales sobre los factores que podían estar implicados en la asistencia a esta población inmigrante, atendiendo especialmente a aquéllos que podían evidenciarse desde el propio sistema sanitario. Esta Tercera Parte se estructura en torno a tres capítulos y un Anexo. El abordaje de estos temas se realiza desde un posicionamiento ético y metodológico claro: es imprescindible que cualquier investigación, más aun si pone su foco de interés en el ser humano, parta de la base de que el conocimiento que podemos generar será siempre falible y provisional y que está sujeto a nuestros propios puntos de vista y convicciones, personales pero también críticos marcados desde los paradigmas de las disciplinas desde las que ejercemos. Y el reto será precisamente intentar aproximarse a la realidad utilizando lo mejor de todas las disciplinas que estén a nuestro alcance (dentro de nuestras propias limitaciones como individuos e investigadores) para tratar de conocerla con la profundidad y amplitud que requiere, observando los fenómenos que se producen, sus consistencias y sus variaciones. La interdisciplinariedad, en éstos y tantos otros temas, pasa de ser intelectualmente aconsejable a ser éticamente imprescindible.


This text is an attempt of approach, from the Anthropology, but by the hand of Epidemiology and Public Health and focused on the public health system, to the possible responses for questions such as: what factors may be involved in the arise of social inequalities concerning health and medical care received by immigrant population and ethnic minorities in our setting. There exist differences in this sense with respect to the rest of the population? And how can our public health system respond to the needs of this populations? And is also the attempt to offer a response to these questions delimited to an specific geographical scope and timeframe: Spain, and specially Catalonia between 2005 and 2010. The text is organized in three sections. The First Section, titled Approach to the studies on health and inequality and divided into two chapters, tackles the main features of certain researchers done within the field of Health as well as within the field of the Anthropology concerning social inequalities in health, diversity and immigration, the analysis of their basis, their possible contributions and limits and the exhibition of the main epidemiological data available as far as health condition and medical care of immigrant population is concerned. The Second Section, titled The access for immigrant population to health care: rights and conditions consists of Chapter 3 and a addendum to this chapter that responds together, to an attempt to go into detail about researching on what factors may be involved in health condition and the use of health services by immigrant population in our setting /environment and in what measure those may be related to the arise of social inequalities in health. The Third Section, titled Towards an empirical contrast of the ideas, shows the results of a research done with the purpose of obtaining data, through an ethnographic work in medical centers and with health professionals that would allow, at the same time, contrast the data shown on the bibliography on the previous sections and, another, test my own first ideas concerning the factors involving the medical care to immigrant population, paying special attention to those that could appear from the health system itself. This Third Part consists of three chapters and an Annex. The approach to these matters is done under a clear ethical and methodological positioning: is essential that any research, particularly if they are focused on the human being, bases on the fact that the knowledge that we can generate will always be fallible and provisional and is subject to our own points of view and convictions, individual but also marked by the paradigms of the disciplines we are working from. And the challenge will be exactly trying to approach to reality using the best of all disciplines at our reach (within our own limits as individuals and researchers) in order to obtain a large and deep and detailed knowledge as it requires, taking into account the phenomenon arisen, its consistencies and variations. Interdisciplinarity in these and many other subjects is not only intellectually advisable but yet ethically indispensable.
