La brucelosis bovina y sus factores de riesgo: evaluación a nivel mundial y en Colombia

Abstract

La brucelosis bovina es una enfermedad zoonótica, con una gran importancia en salud pública y también en el sector ganadero por las pérdidas económicas directas que ocasiona y sobre todo por las restricciones comerciales tanto a los animales infectados como a sus productos. Conocer el estatus sanitario de la enfermedad, su evolución en el tiempo y analizar los factores que influyen en su presentación, se consideran elementos útiles para la adecuada aplicación de medidas sanitarias en un país. En este sentido, se formularon cuatro estudios, cumpliendo con los objetivos planteados. El primer estudio pretendía analizar la evolución de la brucelosis bovina en el mundo Para ello se tomaron datos de 156 países miembros de la OIE en el periodo comprendido entre los años 1996 a 2014. Los países fueron clasificados en tres grupos en función de la situación sanitaria, a) Enzooticos: aquellos países donde la infección estuvo siempre presente o con breves periodos de ausencia (menores de tres años); y No-Enzooticos: aquellos países donde presentó uno o varios periodos de ausencia superiores a tres años y c) Libres: aquellos países donde la enfermedad estuvo ausente durante todo el periodo de estudio. Los países fueron comparados mediante los test no paramétricos de Kruskall-Wallis y de Mann-Whitney y el coeficiente de correlación de Spearman para dos variables cuantitativas. En el último caso, si la relación era significativa, se hizo un modelo de regresión lineal o cúbica para describir mejor la tendencia. Se encontró una relación entre el estatus de la enfermedad y la disponibilidad de recursos económicos, así países con un alto PIB per cápita tendían a estar libres de brucelosis bovina, países con un mayor censo bovino mostraron mayor probabilidad de tener la enfermedad presente. Las regiones más afectadas fueron Centro y Sur América, África y parte de Asia. Los siguientes trabajos se centraron en estudiar la epidemiología de la enfermedad en Colombia. El primero de ellos pretendía estimar la evolución del control de la brucelosis bovina en Colombia en un periodo de siete años (2006-2012). Se realizó un análisis univariado, posteriormente, se realizó un análisis de componentes principales para cada año separadamente. Los resultados indicaron que entre los años de estudio la prevalencia prácticamente no varió (las granjas pasaron de 22% a 23% y la de animales varió entre 4.7% y 4.6% para los años 2006 y 2012, respectivamente). Se definieron tres áreas diferenciadas de la enfermedad de acuerdo al estatus sanitario y la vigilancia. En el segundo estudio, se analizaron los movimientos de las especies animales domésticas de importancia económica en Colombia (bovinos, cerdos, equinos, caprinos y ovinos) durante un periodo de nueve años (2006-2014). Mediante un análisis de redes, se analizó la asociación entre la prevalencia de la enfermedad y el movimiento de animales en el país, posteriormente se desarrollaron un Modelo de Regresión Lineal Múltiple (MLR) y un ANOVA para analizar la relación entre la situación sanitaria y el movimiento de los animales. Los resultados indican el posible papel de los movimientos de équidos en la diseminación de la brucelosis. Finalmente, en el tercer estudio de Colombia, se pretendía identificar los factores de riesgo asociados a la reinfección por Brucella abortus en granjas bovinas de Colombia. Se hizo un estudio de caso-control donde se compararon 98 granjas caso (con reintroducción de la enfermedad) con 93 granjas control (sin reintroducción de la enfermedad). Se realizó un análisis bivariado y un modelo de regresión logística. El reemplazo de animales, la monta natural y el contacto con granjas infectadas o estatus sanitario desconocido, fueron factores que presentaron una relación altamente significativa con la re-infección de las granjas.


Bovine brucellosis is a zoonotic disease, with great importance in public health and in the livestock sector due to the direct economic losses it causes and, specially, due to the commercial restrictions for both the infected animals as well as their products. The knowledge of the health status of the disease, its evolution over time and the factors that influence its presentation is of great importance for the appropriate application of sanitary measures in a country. In this sense, four studies, fulfilling the proposed objectives, were formulated. The first study analyzes the evolution of bovine brucellosis in the world. Data were taken from 156 OIE member countries for the period between the years 1996 and 2014. The countries were classified into three groups according to their sanitary situation a) Enzootic: countries infected, which can have been free of brucellosis but for periods of less than 3 years. b) Non-enzootic: countries where the disease was present, but there was at least a 3-year period without the disease, and c) Free: countries where the disease remained absent during the whole period. The countries were compared by means of the Kruskall-Wallis and Mann-Whitney non-parametric tests, and the Spearman correlation coefficient for two quantitative variables. In this last case, if the relation was significant, a lineal or cubic regression model was made to better describe the tendency. A relationship was found between the status of the disease and the availability of economic resources; thus, countries with a high GDP per capita tended to be free from bovine brucellosis. Countries with a greater bovine census showed a greater probability to have the disease present. The most affected regions were Central and South America, Africa and parts of Asia. The next studies were focused on the epidemiology of the disease in Colombia. The first one intended to estimate the evolution of the bovine brucellosis control in Colombia over a seven years period (2006-2012). A univariate analysis was carried out, then a principal components analysis was performed for each year separately. The results indicated that, during the years of the study, prevalence remained practically unvaried (farms went from 22% to 23%, and animals varied between 4.7% and 4.6% between 2006 and 2012, respectively). Three different areas were defined according to brucellosis status and surveillance. In the second study, movements of the domestic animal species in Colombia (cattle and buffaloes, pigs, horses, goats and sheep) were analyzed over a period of nine years (2006-2014). The association between the disease prevalence and the animal movement was analyzed using a network analysis; subsequently, Multiple Linear Regressions (MLR) and ANOVA test were developed to analyze the relation between the health situation and the movement of animals. The results indicate the possible role of equine movements in the brucellosis spread between departments. Finally, the objective of the third study of Colombian data, was the identification of the risk factors associated with re-infection of Brucella abortus in cattle farms. A case-control study was performed in which 98 case-farms (herds were brucellosis was re-introduced) were compared to 93 control farms (without re-introduction of the disease). A bivariate analysis and a logistic regression model were done. The results indicated that the replacement animals, natural matting and contact with infected farms or with unknown sanitary status, were risk factors for the re-introduction of the disease.
