Abstract

Introducción: La complejidad de la esquizofrenia se encuentra en la combinación de necesidades psiquiátricas, somáticas y sociales. La importancia de controlar el síndrome metabólico (SMet) en las personas con esquizofrenia ha sido reiterada por diversos consensos. Sin embargo, la monitorización es limitada en la práctica clínica. En este contexto, la importancia de la calidad relacionada con la salud (CVRS) y de las necesidades en salud son un un tema importante para los investigadores. No obstante, mientras que las necesidades pueden ser vistas como el punto de partida para el desarrollo de intervenciones para las personas con esquizofrenia, el objetivo final debe ser la mejoría de la calidad de vida (CV). Objetivos: El objetivo general fue analizar la relación entre las necesidades, la CVRS y SMet en una muestra de pacientes con esquizofrenia o trastorno esquizoafectivo atendidos centro de salud mental público en Barcelona/Cataluña/España. Métodos: Los estudios que componen esta tesis tienen un enfoque naturalista. Los diagnósticos se confirmaron mediante la Entrevista Clínica Estructurada del DSM-IV-Eje I. Los participantes fueron entrevistados para obtener datos sociodemográficos, sus estilos de vida, sintomatología psicótica (PANSS), funcionamiento global (GAF), escala CVRS (EQ-5D), necesidades (CAN), medidas antropométricas y análisis de sangre. El SMet fue definido por los Criterios NCEP-ATP-III. mod. (2005). Para el análisis estadístico se realizó el SPSS 20.0. Resultados: La prevalencia del SMet fue de un 36.8%. Los sujetos con SMet tenían mayor peso, IMC, perímetro abdominal, presión arterial, tasa de triglicéridos, tasa de glucosa en ayunas y menor tasa de HDL-colesterol. El SMet también se asoció con la edad, situación laboral inactiva y un peor autocuidado. La EQ-VAS (EQ-5D) mostró que los sujetos con SMet que practican actividad física refieren una mejor autopercepción de salud, lo que refuerza la importancia de promover hábitos saludables y el tratamiento del SMet desde una perspectiva interdisciplinar. El SMet se asoció con historia familiar de diabetes y con una mayor duración de la esquizofrenia. Los individuos con y sin SMet mejoraron las puntuaciones GAF en el seguimiento a 18 meses. Se encontró que los individuos que presentaban SMet en el seguimiento presentaban una mejor calidad de vida subjetiva (no significativo a nivel estadístico). De los pacientes sin SMet, el 23.3% fueron diagnosticados de padecerla a 18 meses del inicio del seguimiento y el 17.6% de los que fueron diagnosticados con SMet en la valoración inicial no presentaron en la evaluación de seguimiento. El número medio de las necesidades totales de los participantes con SMet fue 5.95 (4.26 satisfechas, 1.69 no satisfechas). Los problemas de movilidad (EQ-5D) se asociaron negativamente con el número de las necesidades totales y no satisfechas. La CVRS se relacionó con el número de las necesidades totales y no satisfechas en los pacientes con SMet. El funcionamiento global se asoció significativamente con las necesidades (tanto satisfechas como no satisfechas) de los pacientes sin SMet. La sintomatología psicótica positiva se relacionó con el número de las necesidades totales y necesidades satisfechas, mientras la sintomatología general se relacionó con las necesidades totales en las personas con SMet. Conclusiones: No existe un modelo ideal de atención en salud mental, pero las directrices internacionales deben ser adaptadas a las necesidades locales. El modelo interdisciplinario ayudó a traducir los hallazgos de la investigación en modificación de la clínica. Este hecho tiene implicaciones para la planificación del tratamiento a nivel individual y organizacional. Un análisis de las necesidades tanto físicas como psíquicas podría proporcionar un punto de partida para el diseño de estrategias con el objetivo de mejorar la CV de los pacientes. El plan de futuro es el desarrollo de programas de estilo de vida individualizados en función de sus necesidades específicas. Background: The complexity of schizophrenia lies in the combination of psychiatric, somatic and social needs requiring care. The importance of monitoring metabolic syndrome (MetS) in people with schizophrenia has been reiterated by consensus. However, health screening remains limited in clinical practice. In this context, the importance of health-related quality of life (HRQL) for treatment outcomes has been recognized as an important topic for researchers. The same is true of the needs for care assessment that. However, while needs can be seen as the starting point for developing interventions for people with schizophrenia, the ultimate goal is the improvement of quality of life (QoL). Aims: The general aim was to analyse the relationship between needs, HRQL and MetS in a sample of patients with schizophrenia or schizoaffective disorder attending a public mental health centre in Barcelona/Catalonia/Spain. Methods: The studies that compound this thesis have a naturalistic approach. Interviews were conducted in individual sessions at the mental health centre. The diagnoses were confirmed using the Structured Clinical Interview for the DSM-IV-Axis I disorders. Participants were interviewed to provide sociodemographic data and information on lifestyle habits; anthropometric measurements were taken and metabolic profiles were obtained. MetS was defined by the NCEP-ATP-III mod. Criteria (2005). Severity of psychopathology, global functioning, HRQL and needs for care were evaluated using the Positive And Negative Syndrome Scale, the Global Assessment of Functioning Scale, the Euro-Qol-5 Dimensions and the Camberwell Assessment of Needs, respectively. Statistical analyses were performed using SPSS 20.0. Results: The prevalence of MetS was 36.8%. Subjects with MetS had greater weight, BMI, waist, blood pressure, triglycerides, fasting glucose and a lower HDL-cholesterol. The MetS is also associated with age, inactive employment status and poorer self-care. The EQ-VAS (EQ-5D) shows that subjects with MetS who practice physical activity indicates a better subjective QoL, which remarks the necessity of improving healthy lifestyle and an interdisciplinary treatment of the MetS. MetS was associated with a family history of diabetes and longer duration of illness. Individuals both with and without MetS had improved GAF scores at follow-up. We found that individuals who presented MetS at follow-up had higher HRQL scores than those who did not (not statistically significant). Of the patients who did not have MetS, 23.3% were diagnosed with the condition at follow-up and 17.6% of those who were diagnosed with MetS did not present it at follow-up assessment. The mean number of total needs of the participants with MetS was 5.95 (4.26 met, 1.69 unmet needs). Mobility problems (EQ-5D) were negatively associated with the number of total and unmet needs. HRQL was related to the number of total and unmet needs in patients with MetS. We found that global functioning was significantly associated with needs in patients without MetS. Positive symptomatology was related to the number of total and met needs and general symptomatology with total needs in people with MetS. Conclusions: It is well known that there is no ideal model in mental health care, but international guidelines should be adapted to meet local needs. The interdisciplinary model helped to translate research findings into modifications of the clinical routine. This has implications for treatment planning at the individual and organisational levels. An analysis of both physical and mental needs could provide a starting point for the extension of facilities in the health care system in order to reach the goal of improving HRQL. The future plan is to develop individualized lifestyle programs for individuals with schizophrenia or TEA and MetS based on their specific needs.
