Abstract

Mycobacterium abscessus es un patógeno oportunista emergente perteneciente al grupo de las micobacterias no tuberculosas de crecimiento rápido. El complejo M. abscessus está formado por tres subespecies: M. abscessus subsp. abscessus, M. abscessus subsp. massiliense y M. abscessus subsp. bolletii. M. abscessus causa infecciones cutáneas e infecciones pulmonares crónicas en pacientes con factores predisponentes. M. abscessus es resistente a la mayoría de antibióticos. La correcta identificación a nivel de subespecie es imprescindible para optimizar la pauta terapéutica, al existir diferencias en la resistencia antibiótica entre las tres subepecies. Los macrólidos (claritromicina), constituyen la base del tratamiento. La resistencia a los macrólidos, depende en su mayoría de un gen erm(41) funcional (polimorfismo 28T) presente en la mayoría de las cepas de la subespecie abscessus y en todas las cepas de la subespecie bolletii. La subespecie massiliense presenta un gen erm(41) truncado no funcional. M. abscessus, igual que las demás MNT, tiene un reservorio ambiental des del que infecta al huésped susceptible. El reservorio ambiental de M. abscessus no es conocido con precisión. Se ha descrito la existencia de clones de M. abscessus predominantes en las muestras clínicas de pacientes a nivel mundial. El hecho de que se observen clones predominantes puede deberse a su presencia universal en el ambiente y a que estos clones se seleccionan por su capacidad para infectar al hombre. Se ha propuesto la transmisión interhumana como mecanismo de difusión. El estudio de epidemiología molecular del complejo M. abscessus es importante para poder establecer como se difunde. Hemos estudiado 36 cepas de 33 pacientes de tres hospitales de Barcelona aisladas en un período de 20 años. Para la identificación a nivel de subespecie hemos utilizado el consenso de homología de cuatro genes estructurales; la espectrometría de masas Maldi-ToF; y la secuenciación masiva. Para estudiar la resistencia antibiótica, hemos calculado las CMIs de los fármacos mediante microdilución y E-test. Para los mecanismos moleculares asociados a las resistencia hemos estudiado los genes rrl, erm(41), rrs, MAB_3168c, rpsL, gyrA, gyrB, rplC, rplD y rplV. Para estudiar la epidemiología del complejo hemos utilizado el análisis por VNTR, por MLST y por WGS. Nuestros resultados muestran que actualmente el Maldi-Tof no es capaz de identificar M. abscessus a nivel de subespecie. Por el contrario, la secuenciación de cuatro genes estructurales permite la identificación a nivel de subespecie. EL E-test da resultados menos reproducibles y CMIs más elevadas que la microdilución. Los resultados del estudio de resistencia muestran todas las cepas sensibles a tigeciclina y la mayoría sensibles a los aminoglucósidos, pero mayormente resistentes a los demás antibióticos. Las bases moleculares de la resistencia a los macrólidos se encuentran en la presencia de un gen erm(41) funcional o en la selección de mutaciones en las posiciones 2057, 2058 o 2059 del gen rrl. La mutación A1408G en el gen rrs se corresponde con resistencia a los aminoglucósidos. No hemos encontrado las mutaciones descritas como responsables de resistencia a quinolonas y linezolid. La capacidad discriminativa del VNTR es de 0,045, del MLST de 0,145, y del WGS de 0,034. Hemos observado la existencia de clones predominantes a nivel global. Las agrupaciones moleculares deben ser verificadas mediante la epidemiología de campo. Los clusters detectados incluyen pacientes no relacionados epidemiológicamente. En función de nuestros resultados, la transmisión interhumana tendría un papel anecdótico en la transmisión de M. abscessus. La infección se adquiriría de forma independiente a partir de clones predominantes presentes en el ambiente, explicando la existencia de agrupaciones. El estudio de varias cepas del mismo enfermo espaciadas en el tiempo muestra que se trata de una infección crónica (mismo clon) y no reinfecciones por clones distintos.


Mycobacterium abscessus is an emerging opportunistic pathogen belonging to the rapidly growing non-tuberculous mycobacteria. The M. abscessus complex is diveded into three subspecies: M. abscessus subsp. abscessus, M. abscessus subsp. massiliense and M. abscessus subsp. bolletii. M. abscessus complex cause cutaneous infections and chronic pulmonary infections in patients with predisposing factors. M. abscessus is resistant to most antibiotics. The correct identification at the subspecies level is essential to optimize the therapeutic regimen, as there are differences in antibiotic resistance among the three subepecies. Macrolides (clarithromycin) are the key stone for the treatment. Resistance to macrolides, depends mostly on a functional erm(41) gene (28T polymorphism) present in many strains of the subspecies abscessus and in all strains of the subspecies bolletii, but not in the massiliense subspecies. M. abscessus like other NTM, has an environmental reservoir from which it infects the susceptible host. The environmental reservoir of M. abscessus is not known. Predominant globally circulating clones of M. abscessus have been described and interhuman transmission has been proposed as the most common mechanism for spreading. The fact that predominant clones are observed in clinical samples of patients worldwide may be due to their universal presence in the environment and that these clones are selected for their ability to infect humans. The study of molecular epidemiology of the M. abscessus complex is important to establish how it spreads. We used 36 strains isolated from 33 patients from three hospitals in Barcelona over 20 years. For subspecies level identification we used the consensus homology of four housekeeping genes; mass spectrometry by Maldi-ToF; and whole genome sequence. For the drug susceptibility test we calculated the MICs of the antibiotics using microdilution and E-test. For the molecular mechanisms associated with resistance we have studied the rrl, erm(41), rrs, MAB_3168c, rpsL, gyrA, gyrB, rplC, rplD and rplV genes. For the study of the epidemiology of the complex, we used the VNTR analysis, the MLST analysis and the WGS analysis. Our results show that identification by Maldi-Tof is currently unable to identify the members of the complex at the subspecies level. In contrast, the sequencing of four structural genes allows identification at the subspecies level. The E-test gives less reproducible results and higher MICs than microdilution. The results of the DST show all strains sensitive to tigecycline and most sensitive to aminoglycosides, but mostly resistant to other antibiotics. The molecular basis of resistance to macrolides is found in the presence of a functional erm(41) gene or in the selection of mutations at positions 2057, 2058 or 2059 of the rrl gene. The mutation A1408G in the rrs gene is responsible to high level resistance to aminoglycosides. We have not found the mutations described for resistance to quinolones and linezolid. The discriminative capacity of the VNTR is 0.045, the MLST is 0.145, and the WGS is 0.034. We have observed the existence of predominant clones globally. Molecular clusters should be verified by field epidemiology. The detected clusters include patients not epidemiologically related. Based on our results, interhuman transmission would have an anecdotal role in the transmission of M. abscessus. The infection would be acquired independently from predominant clones present in the environment, explaining the existence of clusters. The study of several strains isolated from the same patient spaced in time shows that the infection is a chronic infection (caused by the same clone) and not reinfections by different clones.
